Appointment Checklist
In order to obtain Live Scan service it is
important you are properly prepared. Below is your Live Scan
appointment checklist:
1. BCII
8016 - Request for Live Scan Service Form:

If you have not been provided
with this form already, you will need to request one from your
referring agency. To ensure your appointment runs as
efficiently as possible, it is imperative you complete
the (BCII 8016) Request For Live Scan Service Form prior
to your arrival.
Be sure you are provided with
the following information so you may properly complete the
request form: |

2. Acceptable Valid Photo
I.D.:
In order to obtain Live Scan services you must
present a valid photo I.D. to the technician. Expired identification
cards will not be accepted. The following are examples of acceptable
photo identification:

3. Social Security Number:
This is a DOJ and FBI requirement. If you do not
provide the Live Scan technician with your Social Security number
you will not receive service.
After Your Appointment:
In most cases, the results from DOJ come back
within 72 hours. Please note, due
to various reasons, results can be delayed. For any significant
delay, DOJ will contact the requesting agency. The results from the
Live Scan fingerprinting are either e-mailed or sent to the
requesting agency listed on the Request for Live Scan Service form.
Once the Live Scan process has been completed at our facility,
please direct all inquiries to the agency handling your employment
or licensing.
